A failing dental bridge can have more than one solution. These three clinical cases show how the condition of the supporting teeth, gums, bone, and existing restoration can lead to very different treatment plans — from preserving natural teeth with individual crowns to rebuilding a failed area with bone grafting and implant support.

An aging lower bridge had begun to fracture and show metal
The patient had a porcelain-fused-to-metal bridge across the lower front teeth that had been in place for decades. Over time, sections of porcelain chipped and fractured, exposing the darker metal framework and affecting both the appearance and function of the restoration.
Although the bridge had served the patient for many years, it was showing significant age-related wear and no longer provided the natural, healthy appearance the patient wanted.
The bridge was failing, but the underlying teeth could still be preserved
Examination showed substantial wear and deterioration of the existing PFM bridge. Chipped porcelain had exposed the metal framework, producing dark areas and an uneven appearance.
Importantly, there was no automatic need to replace the old restoration with another connected bridge or remove the supporting teeth for implants. The natural teeth remained suitable for individual restoration, and the surrounding gum contours were also evaluated to improve the final symmetry.
Bridge removal, laser gum refinement, and individual zirconia crowns
The old bridge was carefully removed while preserving the natural teeth underneath. Laser gum treatment was then used to refine the tissue contours and create better symmetry around the restorations.
Instead of reconnecting the teeth with another traditional bridge, the area was restored with separate zirconia crowns. This eliminated visible metal, created more natural individual tooth shapes, and made the area easier for the patient to floss and maintain.

Why a failing bridge does not always need to be replaced with another bridge
The best replacement for an aging bridge depends on what is happening beneath it. When the supporting teeth remain healthy and structurally sound, those teeth may sometimes be preserved and restored individually rather than being connected again.
This can be useful when the main concerns involve aging materials, fractured porcelain, appearance, or difficulty cleaning beneath a connected restoration. Zirconia crowns can provide a strong, metal-free restorative option for appropriately selected teeth.

Why did this patient receive individual crowns instead of another bridge?

A bridge placed for congenitally missing front teeth had worn down over time
The patient was born without her upper lateral incisors and had relied on a fixed anterior bridge since her teenage years. After many years of service, portions of the porcelain had fractured and the restoration showed clear signs of structural wear.
The bridge had once provided an effective solution for the missing teeth, but the patient now needed an updated restoration that was both more durable and more refined esthetically.
The existing bridge was worn, but healthy natural tooth structure remained
The front bridge showed substantial age-related deterioration and porcelain damage. Because the restoration was in a highly visible part of the smile, replacing it required more than simply duplicating the shape of the old bridge.
The supporting teeth, gum contours, tooth proportions, and relationship between the restorations all needed to be considered. The remaining natural tooth structure was suitable for preservation, allowing the area to be rebuilt without unnecessarily extracting healthy teeth.
Removal of the failing bridge, laser gingivectomy, and custom zirconia restorations
The worn bridge was removed so the supporting teeth and surrounding tissues could be evaluated directly. A conservative laser gingivectomy was then used to refine the gum contours and improve symmetry across the front teeth.
Custom individual zirconia restorations were created to provide a cleaner, more natural result while preserving viable existing tooth structure. The finished restorations corrected the visible deterioration of the old bridge and were designed to blend naturally with the smile.

Why replacing a bridge in the front of the mouth requires careful planning
Anterior bridge replacement has to address function and appearance at the same time. The fit against the supporting teeth matters, but so do tooth length, proportions, gum symmetry, color, translucency, and how the restorations relate to the lips and the patient's overall smile.
For a patient born without certain permanent teeth, the plan can be especially individualized. Failure of an older bridge does not necessarily mean the natural teeth beneath it have also failed.

Why would someone need a bridge at a young age?
Some people are congenitally missing permanent teeth, meaning those teeth never develop. Depending on age, bite, available space, and anatomy, a fixed bridge may be one option for replacing the missing teeth.

Years of chewing forces and normal wear can contribute to porcelain damage. Grinding, clenching, changes in the bite, trauma, and aging restorative materials can also play a role.

A fractured supporting tooth caused the existing bridge to fail
The patient presented with a broken or failing dental bridge after one of the teeth supporting the restoration fractured. Because a conventional bridge depends on its supporting teeth for stability, failure of that tooth compromised the connected restoration as a whole.
The objective was not simply to replace the broken bridge. Treatment needed to address the underlying structural problem and create a healthier, more dependable foundation for the replacement.
The fractured support tooth could no longer predictably hold the bridge
Evaluation showed that the damaged support tooth no longer had a reliable long-term prognosis. The affected portion of the bridge needed to be removed, and the condition of the underlying bone had to be considered before implant-supported reconstruction could move forward.
At the same time, healthy existing dental work was preserved rather than unnecessarily removing or replacing areas that were still functioning well.
Extraction, bone grafting, implant placement, and an implant-supported bridge
The failing portion of the bridge was removed and the fractured support tooth was extracted. Bone grafting was performed to rebuild and preserve the foundation needed for implant treatment.
A dental implant was then incorporated into the reconstruction while healthy surrounding dentistry was maintained whenever possible. After healing, the area was restored with a new implant-supported bridge designed to restore strength, function, and appearance.

Why bone grafting may be necessary after a bridge fails
A traditional bridge relies on natural teeth for support. If one of those teeth fractures, develops severe decay or infection, or can no longer be saved, the connected restoration may lose its foundation.
An implant can replace that source of support, but implants require an adequate volume of healthy jawbone. When bone has been lost — or when an extraction leaves a site needing additional support — grafting may be recommended to rebuild or preserve the area.
